Employees of the California State University system are required to be fully immunized against SARS-CoV-2, the virus that causes COVID-19.
Individuals are considered fully vaccinated for six months after the final dose of a two-dose COVID-19 vaccine (e.g., Pfizer or Moderna) or for two months after receiving a dose of a single-dose vaccine (e.g., Johnson & Johnson/Janssen), without a booster. However, if you completed your two-dose vaccine more than six months before February 28, 2022, or your single-dose vaccine more than two months before February 28, 2022, you will no longer be considered fully vaccinated, per CSU policy definitions, and will need to get a booster.
All employees are requested to submit a vaccine self-certification through the Self-Service functionality in MyHR. To self-certify your vaccination status please login to myHR and navigate to the CSU COVID19 Self Certification link. Alternatively, you may click through the following path:

Within this tool you are required to certify one of the following:
- That you have received an approved COVID-19 vaccination and booster shot, and provide relevant details. In the attachment section, a copy of your COVID-19 vaccination record is to be uploaded.
- That you are seeking a Medical Exemption due to a medical condition (including mental health) for which an approved vaccine presents a significant risk of a serious adverse reaction. Employees seeking a medical exemption must submit the Vaccination Exemption Request for a Medical Reason Questionnaire completed by a certified or licensed healthcare professional.
- That you are seeking a Religious/Deeply Held Belief Exemption due to either
(i) a person's sincerely held religious belief, observance, or practice, which includes any traditionally recognized religion, or
(ii) beliefs, observances, or practices which an individual sincerely holds and that occupy a place of importance in that individual's life, comparable to that of traditionally recognized religions.
- That you will not be accessing SSU or other CSU campus facilities at this time and understand that when this changes, you will abide by the requirements to recertify your vaccination status.
Information submitted through MyHR is confidential. Only members of the COVID Monitoring Team and University Personnel will have access to the information provided.
Employees who are not fully vaccinated and have an approved medical or religious/deeply held belief exemption, will be required to submit weekly COVID-19 test results. Testing may be conducted through community testing sites, your personal health care provider, or through the on-campus testing center. COVID rapid tests and tests self-administered at home will not be accepted
Face masks are required inside all campus buildings except for residential buildings regardless of vaccination status. Face masks are not required outdoors as long as a six-foot physical distance between persons can be maintained. Bandanas and gaiters are not suitable face masks.
Daily Wellness Screening
Each day that an employee plans to be on campus, regardless of vaccination status, they will be required to complete a daily wellness screening.
The on campus COVID-19 testing center is open to all employees and members of their household. The testing center is located in the Denali Gym at the Recreation Center. Access is permitted through the south exterior doors of the facility that lead directly to the Denali Gym.
